Routine referrals: Complete Trak Care request for a Paediatric Supportive and Palliative Care consult.

Referrals are triaged Monday to Friday, every 24 hours. For referrals made on Trakcare the referral status and assessment plan will be updated by the Paediatric Supportive and Palliative Care team following triage and contact will be made with the referrer.
