Palliative and Supportive Care outpatient referrals

All referrals should be agreed with the primary care team and child and/or family prior to referral.

Routine referrals: Complete Trak Care request for a Paediatric Supportive and Palliative Care consult. 

Urgent referrals: 0141 452 4894

Referrals are triaged Monday to Friday, every 24 hours. For referrals made on Trak Care the referral status and assessment plan will be updated by the Paediatric Supportive and Palliative Care team following triage and contact will be made with the refer. 

Please contact the Paediatric Palliative and Supportive Care team to discuss the referral on 0141 452 4894 available Monday to Friday 9am to 5pm. Please be advised that referrals should be agreed with the primary team caring for a child and consent from the child and/or family prior to referral.
